Every child who participates in sports at their school (and some sports organizations) is required to have a pre-participation physical examination. Without it, they will not be allowed to participate. During a sports physical, we assess your child’s overall health and readiness to safely participate in athletics. The goal is to identify underlying conditions that could increase the risk of injuries or health complications.
At Pediatric Partners, we perform the elements of the sports pre-participation physical as part of their annual wellness exam. If you think your child may participate in sports, please print the sports pre-participation physical evaluation forms and bring to your child’s appointment. Please also complete the history portion of the forms prior to the appointment as we need to know the answers to these questions before we can complete the form. If you forget to bring the form, please just let us know when you arrive and we can print a copy for you. We can also complete camp forms for you.
We always complete these forms free of charge when it is done at their annual wellness exam. If we complete the forms at another time, there is a $10 charge. As long as your child has had an annual wellness exam within the past year, we can complete these forms for you.
